Application/ Reapplication

If you feel you would benefit from respite, or need to apply for another year, help yourself to the forms below.  You may fill them in independently, or come to the Huron Respite Network offices at 15 Rattenbury Street East, Clinton.  If a family or individual calls the Respite Coordinator directly, the Coordinator will contact the individual or family within two working days.
 

We also accept referrals from a participating agency or any other agency or support service in the community.  When a referral comes from an agency, the Respite Coordinator will arrange a meeting with the family or the individual.  This meeting may include the referring agency worker. If a family or individual calls the Respite Coordinator directly, the Coordinator will contact the individual or family within two working days.
