Purpose:
The purpose of the Service Enhanced/Transitional Fund is:
i) to provide respite support up to one year at a maximum of $750.00 per
individual/ family.
ii) to allow rapid response to individuals who require emergency respite.
iii) to prevent permanent institutionalization, family breakdown and/or
crisis.
iv) to support the transition to ongoing plans for transition aged
individuals and those with aging parents e.g. counseling services, and
school to work initiatives.
OR
v) to support an identified need by the individual and/or family for respite
services that may be ongoing e.g. to improve quality of life and/or to
maintain supports within the family.
Eligibility:
- developmental disability and 18 years of age or older.
- in transition and/or a need for ongoing respite has been identified by a
Servicing Agency and/or family.
- individual resides in Huron County.
- other resources for respite services have been exhausted before attempting
to access this fund.
- is not eligible for respite services from another service system e.g.
Corrections, Health, Education, and Long-Term Care.
- is not receiving funding through an existing service provider for respite
supports.
Process:
Any individual, agency case manager, community advocate, or family member
may apply in writing for assistance to the Huron Respite Network to access
this fund. The Respite Coordinator will present the request in writing to
the Huron Respite Network Selection Committee who will then review,
evaluate, approve or deny the written request.
The written request must include:
- purpose of the request for funding to determine its urgency.
- the current situation, needs of the individual, current services etc.
- expected outcomes and description of service(s) to be used.
- an individualized plan including: time frame, projected costs, pertinent
goals, and ideas for long-term support.
The Selection Committee Will:
i) discuss the application with the presenter and determine if it is a valid
request and meets the eligibility requirements
ii) will then approve or deny the plan of action and ensure that dollars are
available to meet the prioritized requests.
The Respite Coordinator Will:
i) inform applicant of approval or denial of their request; send a
confirmation
package which includes a Service Agreement outlining the amount and purpose
of the funding.
ii) record and follow up on the use of funds and personal outcomes listed in
the
plan.
iii) ensure the individual or agency making the referral to the Service
Enhanced/
Transitional Fund is responsible for case management of the funds provided
by the Huron Respite Network.
iv) monitor and document the annual utilization of allocated funds and
provide a
quarterly report to the Huron Respite Network Advisory Committee.
Conflict of Interest:
Committee Members will declare a conflict of interest in a situation whereby
that Member or Agency that he/she represents has an interest in a proposed
or existing contract or transaction. After making such a declaration, that
Member/Agency shall not vote on the contract or transaction, nor be counted
in the quorum of members.
